Budget bill awaiting Pres Clinton's signature includes two health insurance options that give Medicare beneficiaries greater freedom to choose doctors and hospitals, but it also exposes elderly patients to much higher costs; one option allows Medicare beneficiaries to sign private contracts with doctors for any medical service at any price, the patient paying the entire cost with his own money or private insurance; second option allows Medicare beneficiaries to enroll in private health plans that give patients freedom to choose their doctors and hospitals; Medicare would make fixed monthly payments to such health plans, and there would be no limit on premiums the plans can charge patients (M)
